The Daily Express claims AC Milan are planning a swap deal involving striker Carlos Bacca and Chelsea midfielder Cesc Fabregas.
Bacca was heavily linked with a move to the Premier League over the summer, with West Ham reportedly interested.
And Milan are preparing for his departure, with Fabregas thought to be their top target should the Colombian depart.
The Blues have had a fairly strong start to the 2016/17 season under new manager Antonio Conte, though their unbeaten run ended with a 2-1 home defeat to Liverpool on Friday night.
Diego Costa scored again to make it five goals in five games under Conte, but the Italian is understood to be keen on signing another top class striker.
Chelsea have little in the way of backup to the in-form Spaniard, with Michy Batshuayi yet to work his way into Conte’s plans, and with the likes of Loic Remy, Radamel Falcao and Alexandre Pato all leaving in the summer.
